Job Description

We are seeking a dedicated Float Clinical Care Coordinator to support nursing homes by assisting, substituting, and mentoring MDS Coordinators as needed. Position Details:

Schedule: Full-time, flexible hours

Travel: May involve travel between client facilities (mileage reimbursement available)

Work Setting: Hybrid (remote and on-site, depending on experience)

Key Responsibilities:

Clinical Support & Training:

Assist nursing facilities and MDS Coordinators in completing resident assessments in accordance with state and facility policies.

Conduct clinical record reviews to ensure accurate and consistent documentation.

Train and mentor MDS Coordinators on assessment procedures and documentation standards.

Provide hands-on support to address facility-specific needs.

Intervention & Auditing:

Identify areas of concern within facilities and collaborate to develop and implement effective action plans.

Conduct interventions, audits, and follow-ups based on facility requirements.

Remote & On-Site Flexibility:

Semi-remote work is available for candidates with over five years of MDS experience and Pennsylvania Case-Mix expertise.

Qualifications:

Active Registered Nurse (RN) license in Pennsylvania.

Minimum of 1 year of experience as an RN Assessment Coordinator with expertise in the MDS process.

Flexibility to travel between multiple locations as needed.
